Is My Family Eligible For Services?

Guilford Child Health accepts Medicaid, Health Choice and other insurance policies. If you do not have health insurance or Medicaid, GCH will see you if your family’s income is at or below 200 percent of the federal poverty limit. GCH offers a sliding fee scale for those without insurance and without Medicaid.

To allow us to serve you better, please bring the following information
to each appointment:

• Medicaid or Insurance Card
• Child's Immunization Record
• Child's Current Medications
• Copy of Child's Medical Records from Current Medical Provider (initial appointment)
